ABSTRACT

Introdução: As doenças cardiovasculares são uma grande preocupação para a saúde mundial. A falta de seguimento terapêutico dessas doenças está associada com o alto risco de eventos cardiovasculares. Compreender os elementos que motivam essa adesão pode impulsionar a elaboração de intervenções mais efetivas. Objetivo: Identificar os elementos reguladores da motivação ao seguimento terapêutico das doenças cardiovasculares. Metodologia: Trata-se de uma revisão integrativa segundo as diretrizes de Whitemore e Knafl. Realizou-se a seleção dos artigos entre dezembro de 2020 e janeiro de 2021 nas bases de dados: LILACS, SciELO, MEDLINE e Cochrane Library, com recorte temporal de 2011 a 2021, nos idiomas português, inglês e espanhol. Foram excluídos os artigos de revisão, reflexão, guidelines, protocolos de pesquisa e artigos repetidos. 19 estudos foram selecionados. Resultados: Estudos oriundos de 12 países, mostraram os reguladores de motivação ao tratamento das doenças cardiovasculares em cinco categorias: suporte social, saúde física, autodeterminação, reguladores psicoemocionais e sistemas de cuidado. Conclusão: A motivação apresenta regulações complexas, que mobilizam e potencializam o comportamento humano. Os resultados podem direcionar a assistência de enfermagem e as ações intencionais que influenciem no seguimento terapêutico de doenças cardiovasculares, melhorando os resultados de saúde.


Introducción: Las enfermedades cardiovasculares son una preocupación importante para la salud mundial. La falta de seguimiento terapéutico de estas enfermedades se asocia con un alto riesgo de eventos cardiovasculares. Comprender los elementos que motivan esta adherencia puede impulsar el desarrollo de intervenciones más efectivas. Objetivo: Identificar los elementos reguladores de la motivación para el seguimiento terapéutico de las enfermedades cardiovasculares. Métodos: Se trata de una revisión integradora, siguiendo los lineamientos de Whitemore y Knafl. La selección de artículos se realizó entre diciembre de 2020 y enero de 2021 en las bases de datos LILACS, SciELO, MEDLINE y Cochrane Library. El marco temporal establecido para la selección fue de 2011 a 2021 y los idiomas fueron portugués, inglés y español. Se excluyeron artículos de revisión, reflexiones, guías, protocolos de investigación y artículos repetidos. Se seleccionaron 19 artículos. Resultados: Estudios de 12 países mostraron los reguladores de la motivación para el tratamiento de enfermedades cardiovasculares en cinco categorías: apoyo social, salud física, autodeterminación, reguladores psicoemocionales y sistemas de atención. Conclusión: En esencia, la motivación tiene regulaciones complejas que movilizan y mejoran el comportamiento humano. Los resultados pueden orientar los cuidados de enfermería y las acciones intencionales que influyen en el seguimiento terapéutico de las enfermedades cardiovasculares, mejorando los resultados de salud.


Introduction: Cardiovascular diseases are a major concern for global health. The lack of therapeutic follow-up for these diseases is associated with a high risk of cardiovascular events. Understanding the elements that motivate this adherence can prompt the development of more effective interventions. Objective: To identify the regulatory elements that regulate the motivation for the therapeutic follow-up of cardiovascular diseases. Methods: This is an integrative review following Whitemore and Knafl's guidelines. The articles were selected between December 2020 and January 2021 in the databases: LILACS, SciELO, MEDLINE and Cochrane Library, with a time frame from 2011 to 2021, in Portuguese, English and Spanish. Review articles, reflections, guidelines, research protocols, and repeated articles were excluded. 19 articles were selected. Results: Studies from 12 countries showed the regulators of motivation for the treatment of cardiovascular diseases in five categories: social support, physical health, self-determination, psycho-emotional regulators and care systems. Conclusion: In essence, motivation has complex regulations that mobilize and enhance human behavior. The results can guide nursing care and intentional actions that influence the therapeutic follow-up of cardiovascular diseases, improving health outcomes.

Abstract Objective Given the divergence of opinions on the need for complementary tests such as ultrasonography (US) and electroneuromyography (ENMG) for the diagnosis of carpal tunnel syndrome (CTS), we aimed to elucidate which of them presents greater accuracy for the confirmation of the presence or not of this condition. Methods A total of 175 patients from a hand surgery outpatient clinic were clinically evaluated, and the results of clinical trials (Tinel, Phalen and Durkan), US (normal or altered), and ENMG (normal, mild, moderate and severe) were noted, crossed, and submitted to a statistical analysis to verify the agreement between them. Results with the sample had a mean age of 53 years, with a prevalence of female patients (159 cases). Of the patients with positive clinical test, 43.7% had normal US and 41.7% had no alterations on the ENMG. Negative results were found on the Tinel in 46.9%, on the Phalen in 47.4%, and on the Durkan in 39.7%. In the crossing between the results of the ENMG and those of the other diagnostic methods, there was little statistical agreement between them. Conclusion There was no agreement between the results of the clinical examinations, the US and the ENMG in the diagnosis of CTS, and there is no clinical or complementary examination for CTS that accurately determines the therapeutic approach. Resumo Objetivo Diante da divergência sobre a necessidade de exames complementares, como ultrassonografia (US) e eletroneuromiografia (ENMG) para o diagnóstico da síndrome do túnel do carpo (STC), objetivamos elucidar qual deles apresenta maior precisão na confirmação da presença ou não desta afecção. Métodos Um total de 175 pacientes de um ambulatório de cirurgia da mão foram avaliados clinicamente, e os resultados dos testes clínicos (Tinel, Phalen e Durkan), da US (normal ou alterada) e da ENMG (normal, leve, moderada e grave) foram anotados, cruzados, e submetidos a análise estatística para verificar a concordância entre eles. Resultados A idade média da amostra era de 53 anos, sendo prevalente o sexo feminino (159 casos). Dos pacientes com teste clínico positivo, 43,7% apresentavam US normal, e 41,7%, ENMG sem alterações. Foram encontrados resultados negativos no Tinel em 46,9% no Phalen em 47,4%, e no Durkan em 39,7%. No cruzamento entre a ENMG e os demais métodos diagnósticos, houve pouca concordância estatística. Conclusão Não houve concordância entre os resultados dos exames clínicos, da US e da ENMG no diagnóstico da STC, e não há exame clínico ou complementar para STC que determine a conduta terapêutica com precisão. ABSTRACT

Objetivo: Caracterizar e diferenciar as técnicas de escultura dental, bem como descrever as etapas da ceroplastia de dentes. Revisão Bibliográfica: As principais técnicas encontradas foram: Técnica de escultura regressiva de dentes isolados em cera; técnica progressiva; técnica de ceroplastia em dentes articulados no manequim; enceramento progressivo em molde de gesso; enceramento progressivo com a oclusão dente-a-dente, cúspide-fossa; técnica de escultura de dentes articulados (restaurações unitárias); técnica de escultura de dentes articulados (restaurações múltiplas). No geral, o material e instrumental utilizados nas práticas de escultura são definidos pelo corpo docente do componente curricular Escultura Dental, e alguns desses diferem de uma universidade para outra, assim como o passo a passo seguido. Para o alcance da completa e correta escultura, faz-se necessário seguir a sequência evitando ultrapassar uma etapa sem antes haver cumprido com plena observância dos detalhes o passo imediatamente anterior. Considerações finais: Não houve uma técnica de escultura dental padrão a ser aplicada, entretanto acredita-se que a técnica de ceroplastia em dentes articulados no manequim destaca-se na prática laboratorial, pois os dentes a serem esculpidos precisaram respeitar o tamanho, a relação de oclusão, a relação interdental e o alinhamento, de forma a simular um procedimento clínico.


Objective: To characterize and differentiate dental carving techniques, as well as describe the stages of wax-up of teeth. Bibliographic Review: The main techniques found were: Technique of regressive sculpture of isolated teeth in wax; progressive technique; wax-up technique on articulated teeth on the dummy; progressive waxing in plaster mold; progressive waxing with tooth-to-tooth, cusp-fossa occlusion; articulated teeth sculpting technique (single restorations); articulated teeth sculpting technique (multiple restorations). In general, the material and instruments used in sculpting practices are defined by the faculty of the Dental Sculpture curricular component, and some of these differ from one university to another, as well as the step by step followed. In order to achieve a complete and correct sculpture, it is necessary to follow the sequence, avoiding going beyond a stage without first having completed the immediately preceding step in full observance of the details. Final considerations: There was no standard dental sculpting technique to be applied, however, it is believed that the wax-up technique on articulated teeth on the dummy stands out in laboratory practice, as the teeth to be sculpted needed to respect the size, the relationship of occlusion, interdental relationship and alignment, in order to simulate a clinical procedure.


Objetivo: Caracterizar y diferenciar las técnicas de tallado dental, así como describir las etapas del encerado de los dientes. Revisión Bibliográfica: Las principales técnicas encontradas fueron: Técnica de escultura regresiva de dientes aislados en cera; técnica progresiva; técnica de encerado en dientes articulados en el maniquí; encerado progresivo en molde de yeso; encerado progresivo con oclusión cúspide-fosa diente a diente; técnica de esculpido de dientes articulados (restauraciones individuales); Técnica de esculpido de dientes articulados (restauraciones múltiples). En general, los materiales e instrumentos utilizados en las prácticas de escultura son definidos por la facultad del componente curricular de Escultura Dental, y algunos de estos difieren de una universidad a otra, así como el paso a paso seguido. Para lograr una escultura completa y correcta, es necesario seguir la secuencia, evitando pasar de una etapa sin haber completado antes el paso inmediatamente anterior con total atención a los detalles. Consideraciones finales: No existía una técnica de esculpido dental estándar a aplicar, sin embargo, se cree que la técnica de encerado sobre dientes articulados sobre el maniquí se destaca en la práctica de laboratorio, ya que los dientes a esculpir debían respetar el tamaño, la relación de oclusión, relación interdental y alineación, con el fin de simular un procedimiento clínico. ABSTRACT

ABSTRACT Brazil is a great source of arbovirus diversity, mainly in the Amazon region. However, other biomes, especially the Atlantic Forest, may also be a hotspot for emerging viruses, including Bunyaviruses (Negarnaviricota: Bunyavirales). For instance, Vale do Ribeira, located in the Southeastern region, has been widely studied for virus surveillance, where Flavivirus, Alphavirus and Bunyaviruses were isolated during the last decades, including Bruconha virus (BRCV), a member of Orthobunyavirus genus Group C, in 1976. Recently, a new isolate of BRCV named Span321532 was obtained from an adult sentinel mouse placed in Iguape city in 2011, and a full-length genome was generated with nucleotide differences ranging between 1.5%, 5.3% and 5% (L, M and S segments, respectively) from the prototype isolated 35 years earlier. In addition, each segment placed BRCV into different clusters, showing the high variety within Bunyavirales. Although no evidence for reassortants was detected, this finding reiterates the need for new surveillance and genomic studies in the area considering the high mutation rates of arbovirus, and also to identify the hosts capable of supporting the continuous circulation of Orthobunyavirus.

ABSTRACT

Objetivo:Compreender a percepção de gestantes noacompanhamento pré-natal durante a pandemia da COVID-19.Métodos:Estudo analítico com abordagem qualitativa,realizado com onze gestantes atendidas porduas equipes de Estratégia Saúde da Família do município de Colinas, Maranhão. A coleta de dados foi realizada mediante entrevistas semiestruturadas gravadas e após transcritas. Os dados foram processados com o auxílio do softwareIRaMuTeQpor meio da Classificação Hierárquica Descendente e analisados por meio de interpretação de sentidos.Resultados:O processamento dos dados resultou em seis classes que, após a interpretação de sentidos,deram origem àduas categorias: Percepções ao atendimento pré-natal durante a pandemia da COVID-19e Experiências,aprendizados e cuidados na gestação diante da pandemia.Conclusão:A aquisição de medidas para o controle da transmissão do vírus mostrou-segeradora de dificuldades na adesão ao pré-natal, afetando a qualidade do atendimento em questão. O distanciamento social e a redução do tempo de permanência nas consultas afetaram a relação estabelecida entre profissional e gestante, com isso a atenção foi prejudicada.


Objective:To understand pregnant women's perception aboutprenatal monitoring during the COVID-19 pandemic.Methods:An analytical study with a qualitative approach conducted with elven pregnant women assisted bytwo Family Health Strategy teams from the municipality of Colinas, Maranhão. Data collection was carried out by means of semi-structured interviews, which wererecorded and later transcribed. The data were processed with the aid of the IRaMuTeQsoftware program by means of Descending Hierarchical Classification and analyzed through interpretation of meanings. Results:Data processing resulted in six classes which, after interpreting the meanings,gave rise to two categories: Perceptions about prenatal care during the COVID-19 pandemic; and Experiences, lessons learned and care measures in pregnancy during the pandemic.Conclusion:The imposition of measures to control transmission of the virus proved tocreate difficulties in adherence to prenatal care, affecting quality of the assistance provided. Social distancing and reduction in the length of the consultations affected the relationship established between the professionals and the pregnant women, therefore impairing care.

Abstract Objective The incidence of traumatic brachial plexus injuries has been increasing considerably in Brazil, mainly due to the increase in the number of motorcycle accidents. The aim of the present study is to evaluate the sensitivity and specificity of magnetic resonance imaging (MRI) in the diagnosis of brachial plexus avulsion lesions, comparing it with the findings of physical and intraoperative examination. Methods A total of 16 patients with brachial plexus injury were prospectively evaluated and treated at the hand surgery outpatient clinic from our service. All patients underwent MRI of the brachial plexus, and the findings were inserted on a table, as well as the physical examination data, and part of the patients had the plexus evaluated intraoperatively. Results In the present study, the accuracy of MRI in the identification of root avulsion was 100%, with 100% sensitivity and specificity when comparing imaging with surgical findings. Conclusion Magnetic resonance imaging showed high sensitivity and specificity, confirmed by intraoperative findings, which allows considering this test as the gold standard in the diagnosis of avulsion in traumatic brachial plexus injuries.


Resumo Objetivo A incidência de lesões traumáticas do plexo braquial vem aumentando consideravelmente no Brasil, principalmente devido ao aumento do número de acidentes de motocicleta. O objetivo do presente estudo é avaliar a sensibilidade e a especificidade da ressonância magnética (RM) no diagnóstico das lesões por avulsão do plexo braquial, comparando com os achados do exame físico e do intraoperatório. Métodos Foram avaliados prospectivamente 16 pacientes com lesão do plexo braquial atendidos no ambulatório de cirurgia da mão de nosso serviço. Todos os pacientes foram submetidos ao exame de RM do plexo braquial e os achados foram inseridos em uma tabela, assim como os dados do exame físico, e parte dos pacientes teve o plexo avaliado intraoperatoriamente. Resultados No presente estudo, a acurácia da RM na identificação de avulsão de raízes foi de 100%, com 100% de sensibilidade e especificidade comparando-se achados da imagem e cirúrgicos. Conclusão A RM mostrou alta sensibilidade e especificidade, confirmadas por achados intraoperatórios, o que permite considerar este exame como padrão outro no diagnóstico de avulsão nas lesões traumáticas do plexo braquial.

ABSTRACT

Abstract Objective This is an anatomical study of C4 and C5 roots for nerve transfers in upper brachial plexus injuries, with surgical technique demonstration. Methods Fifteen brachial plexuses from both male and female cadavers were dissected. Morphological features of C4 and C5 roots were recorded and analyzed, followed by a neurotization simulation. Results In all dissections, C4 and C5 roots morphological features allowed their mobilization and neurotization with no need for a nerve graft. The surgical technique spared important regional nerve branches. Conclusion Based on these data, we conclude that C4-C5 nerve transfers are feasible and result in no additional neurological deficit in upper brachial plexus injuries.


Resumo Objetivo Estudo anatômico das raízes usadas na transferência nervosa de C4 para C5 nas lesões altas do plexo braquial, com demonstração da técnica cirúrgica. Métodos Dissecção de 15 plexos braquiais de cadáveres de ambos os sexos, registro e análise das características morfológicas das raízes de C4 e C5 e simulação de neurotização. Resultados As características morfológicas encontradas nas raízes de C4 e C5 em todas as dissecções permitiram a mobilização das mesmas e a realização de uma neurotização sem a necessidade de usar enxerto nervoso. A técnica cirúrgica permitiu preservar ramos nervosos importantes na região abordada. Conclusão Com base nos dados encontrados no presente estudo, podemos concluir que é possível realizar a transferência entre C4 e C5 sem provocar déficit neurológico adicional nas lesões altas de plexo braquial.

ABSTRACT

Abstract Odontomas are the most common tumor-like (hamartoma) odontogenic lesions in the gnathic bones. There are two main types: complex odontoma which is composed mainly of amorphous calcification, and compound odontoma which is characterized by irregular tooth-like structures. Odontomas are generally asymptomatic and in most cases involve only one quadrant of the maxilla or mandible. The treatment of choice is surgical removal. The aim of the present study is to report a rare case of multiple complex odontomas affecting the mandible and to review the literature on the clinical and pathological features of these tumors.


Resumo Odontomas são as lesões odontogênicas mais comuns nos ossos gnaticos mais comuns. Existem dois tipos principais: odontoma complexo que é composto principalmente de calcificação amorfa, e odontoma composto que é caracterizado por estruturas irregulares semelhantes à dentes. Odontomas são geralmente assintomáticos e, na maioria dos casos, envolvem apenas um quadrante da maxila ou mandíbula. O tratamento escolhido é a remoção cirúrgica.


Resumen Los odontomas son las lesiones odontogénicas más comunes en los huesos gnáticos más comunes. Hay dos tipos principales: odontoma complejo, que se compone principalmente de calcificación amorfa, y odontoma compuesto, que se caracteriza por estructuras irregulares similares a dientes. Los odontomas suelen ser asintomáticos y, en la mayoría de los casos, afectan solo un cuadrante del maxilar o la mandíbula. El tratamiento elegido es la extirpación quirúrgica.

Abstract Objective To evaluate elbow flexion in children with obstetric brachial plexus paralysis submitted to Oberlin transfer. Methods Retrospective study with 11 patients affected by paralysis due to labor who did not present spontaneous recovery from elbow flexion until 12 months of life, operated between 2010 and 2018. Results The children were operated between 5 and 12 months of life, with a mean of 7.9 months, and the mean follow-up time was 133.2 months, ranging from 37 to 238 months. Six patients (54.5%) presented a degree of muscle strength ≥ 3, measured by the strength scale of the Medical Research Council (MRC) and, according to the active movement scale (AMS), 5 patients obtained a score of ≥ 5. A negative correlation was identified between the AMS and the Narakas classification (r = -0.509), as well as between the strength scale (MRC) and the Narakas classification (r = -0.495). A strong positive correlation was observed (r = 0.935) between the AMS and the MRC demonstrating that the higher the score on the movement scale, the higher the score on the muscle strength scale. Conclusion The Oberlin surgery is a possible option for recovery of elbow flexion in children with neonatal plexopathy, demonstrating, however, very heterogeneous results, even in the long-term follow-up.


Resumo Objetivo Avaliar a flexão do cotovelo em crianças portadoras de paralisia obstétrica do plexo braquial submetidas à transferência de Oberlin. Métodos Estudo retrospectivo com 11 pacientes acometidos por paralisia decorrente do trabalho de parto e que não apresentaram recuperação espontânea da flexão do cotovelo até os 12 meses de vida, operados entre 2010 e 2018. Resultados As crianças foram operadas entre os 5 e 12 meses de vida, com média de 7,9 meses e o tempo médio de seguimento foi de 133,2 meses, variando de 37 a 238 meses. Seis pacientes (54,5%) apresentaram grau de força muscular ≥ 3, medido pela escala de força do Medical Research Council (MRC), e, pela escala de movimentação ativa (Active Momement Scale [AMS]), 5 pacientes obtiveram pontuação ≥ 5. Foi identificada correlação negativa entre a AMS e a classificação de Narakas (r = -0,509), bem como entre a MRC e a classificação de Narakas (r = -0,495). Já entre a AMS e a MRC, foi observada forte correlação positiva (r = 0,935), demonstrando que quanto maior a pontuação na escala de movimento, maior será a pontuação na escala de força muscular. Conclusão A cirurgia de Oberlin apresenta-se como uma opção possível para a recuperação da flexão do cotovelo em crianças com plexopatia neonatal; no entanto, demonstra resultados bastante heterogêneos, mesmo no seguimento a longo prazo.

Resumo O objetivo desta pesquisa qualitativa foi investigar as concepções teóricas dos trabalhadores sobre o grupo bem como analisar os fatores impulsores e restritivos da prática terapêutica grupal nos Centros de Atenção Psicossocial (CAPS). Participaram do estudo 66 trabalhadores de CAPS e ambulatórios da Rede de Atenção Psicossocial (RAPS) de 23 municípios do estado de Goiás. A coleta de dados se deu por meio de questionário estruturado e entrevista grupal registrada em gravação de áudio e fotografias. Da análise temática dos dados emergiram o conceito do que o grupo é e do que o grupo não é. Tanto no eixo das forças impulsoras, quanto das forças restritivas, as categorias foram organizadas em três blocos de análise: aspectos relacionados ao serviço, aos profissionais e aos usuários. Aspectos relacionais, estruturais, terapêuticos e a competência profissional para a coordenação de grupo se integram de modo antagônico, complementar e indissociável para apreensão da realidade estudada. Conclui-se que evidenciar os aspectos restritivos que precisam ser reconhecidos e aprimorados bem como os impulsores que precisam ser mantidos e potencializados pode contribuir ativamente para ampliar a capacidade terapêutica do uso da tecnologia grupal no contexto da saúde mental.


Abstract This qualitative research aimed to investigate workers' theoretical conceptions regarding group work and analyze the driving and restrictive factors of group therapeutic practice in Psychosocial Care Centers (CAPS). Sixty-six workers from CAPS and outpatient clinics from the Psychosocial Care Network from twenty-three municipalities in the State of Goiás participated in the study. Data were collected through a structured questionnaire and a group interview audio-recorded and documented with photographs. The concept of what the group is and is not emerged from the thematic analysis of the data. The categories were organized into three analysis blocks concerning driving and restrictive forces: service-related, professional-related, and user-related. Relational, structural, and therapeutic aspects and professional competence for group coordination are integrated in an antagonistic, complementary, and inseparable way to apprehend the studied reality. We conclude that highlighting the restrictive aspects that must be recognized and improved and the drivers that must be maintained and enhanced can actively contribute to expanding the therapeutic capacity related to the use of group technology in mental health.

ABSTRACT Background: Despite their worldwide occurrence, the distribution and role of insect-specific flaviviruses (ISFs) remain unclear. Methods: We evaluated the presence of ISFs in mosquitoes collected in São Paulo, Brazil, using reverse transcription and semi-nested polymerase chain reaction (PCR). Some of the positive samples were subjected to nanopore sequencing. Results: Twelve mosquito pools (2.8%) tested positive for flavivirus infection. Nanopore sequencing was successfully performed on six samples. Phylogenetic analysis grouped these sequences into genotype 2 of Culex flavivirus (CxFV). Conclusions: The identification of CxFV genotype 2 at new locations in São Paulo highlights the importance of understanding the role of ISFs in mosquito vector competence.

Resumo: Introdução: A empatia, tida como uma das características mais marcantes dos grandes profissionais médicos, é o elemento central da relação médico-paciente e do cuidado centrado na pessoa. Objetivo: Este estudo teve como objetivo investigar como se manifestam os níveis de empatia em estudantes de Medicina ao longo da graduação. Método: Trata-se de um estudo de natureza quantitativa, analítica, observacional e transversal, realizado numa instituição privada de ensino superior, situada no Nordeste do Brasil. A pesquisa se deu por meio da aplicação da Escala Jefferson de Empatia Médica - versão para estudantes (JSPE-vs) e da correlação dos dados obtidos na escala com o período da graduação e o perfil sociodemográfico dos estudantes, a fim de verificar quais correlações se mostram significativas para a expressão dos níveis de empatia dos estudantes, bem como se há erosão dela durante a formação. A coleta de dados ocorreu entre os meses de outubro e novembro de 2020. Contou com 193 participantes entre ingressantes, intermediários e concluintes do curso de Medicina. A amostragem utilizada foi por acessibilidade e conveniência. Resultados: A pontuação média global do nível de empatia no conjunto de todos os participantes do estudo (n = 193) foi de 123,56 ± 11,73. Quanto ao período, obteve-se o seguinte resultado: ingressantes = 124,78 ± 9,85, intermediários = 124,00 ± 11,87 e concluintes = 120,63 ± 13,57. Não se verificou diferença estatística entre os escores global ou por fator na comparação entre os três grupos estudados. E na correlação da JSPE-vs com o perfil sociodemográfico, as variáveis sexo feminino e motivo de escolha do curso por vocação foram preditoras de escores maiores de empatia. Conclusão: Não se evidenciou erosão dos níveis de empatia nos estudantes de Medicina ao longo da graduação, e os discentes do sexo feminino e aqueles que escolheram o curso por se sentirem vocacionados para tal mostraram níveis de empatia significativamente maiores. Mais estudos sobre esse tema são fundamentais, tendo em vista a importância de uma postura técnico-científico-humanística equilibrada para o exercício de uma medicina de excelência.


Abstract: Introduction: Empathy, considered one of the most remarkable characteristics of great medical professionals, is the central element of the doctor-patient relationship and of person-centered care. Objective: To investigate how levels of empathy are manifested in medical students throughout their undergraduate course. Method: This is a quantitative, analytical, observational and cross-sectional study, carried out in a private higher education institution, located in the northeast of Brazil. The research was carried out by applying the Jefferson Scale of Empathy-Student version (JSE-S) and the correlation of the data obtained in the scale with the undergraduate school period and sociodemographic profile of the students, aiming to verify which correlations were significant for the expression of students' empathy levels, as well as whether there is an erosion during training. Data collection took place between the months of October and November 2020. The study assessed 193 participants, including students attending the first year, an intermediate year and last year of the medical course. The sampling method used was by accessibility and convenience. Results: The global average score of the level of empathy in the group of all study participants (n = 193) was 123.56 ± 11.73. Whereas, by period, it was 124.78 ± 9.85 for first-year students, 124.00 ± 11.87 for intermediate-year students and 120.63 ± 13.57 for last-year students. There was no statistical difference between global scores or by psychometric factor when comparing the three studied groups. As for the correlation of JSE-S with the sociodemographic profile, the variables female gender and reason for choosing the course due to vocation were predictors of higher empathy scores. Conclusion: There was no evidence of empathy erosion regarding the levels of empathy among medical students throughout their undergraduate course, and female students and those who chose the course because they felt they had a vocation for Medicine showed significantly higher levels of empathy. More studies on this topic are essential, considering the importance of a balanced technical-scientific-humanistic posture to enable a medical practice of excellence.

ABSTRACT Hand-foot-and-mouth disease (HFMD) is a highly contagious viral disease commonly associated to Enteroviruses (EV). During 2018, Brazil faced massive HFMD outbreaks spread across the country. This study aimed to characterize the EV responsible for the HFMD outbreak that occurred in Paraiba State, Brazilian Northeastern region, in 2018, followed by a phylogenetic analysis to detail information on its genetic diversity. A total of 49 serum samples (one from each patient) collected from children ≤ 15 years old, clinically diagnosed with HFMD were tested for EV using conventional RT-PCR and RT-qPCR. EV infection was confirmed in 71.4% (35/49) of samples. The mean and median ages were 1.83 years and one year old, respectively. Twenty-two EV-positive samples were successfully sequenced and classified as EV-A species; 13 samples were also identified with the CV-A6 genotype. The phylogenetic analysis (VP1 region) of three samples revealed that the detected CV-A6 strains belonged to sub-lineage D3. The CV-A6 strains detected here clustered with strains from South America, Europe and West Asia strains that were also involved in HFMD cases during the 2017-2018 seasons, in addition to the previously detected Brazilian CV-A6 strains from 2012 to 2017, suggesting a global co-circulation of a set of different CV-A6 strains introduced in the country at different times. The growing circulation of the emerging CV-A6 associated with HFMD, together with the detection of more severe cases worldwide, suggests the need for a more intense surveillance system of HFMD in Brazil. In addition, this investigation was performed exclusively on serum samples, and the analysis of whole blood samples should be considered and could have shown advantages when employed in the diagnosis of enteroviral HFMD outbreaks.

ABSTRACT Objective: To evaluate the volar cortical angle (VCA), the variation in lateral and intermediate columns, the shape and pattern of the watershed line (WL) of the distal radius and its correlation with locking volar plates available. Methods: 27 human cadaveric radial bones of the Department of Morphology and nine locking volar plates of six different companies were analyzed. VCA were measured from lateral and intermediate columns and their corresponding values on plates, as well as comparing the relief of radius and plates. In the WL analysis, we compared the standard format found on the radius and its equivalent on plates and divided into four types: biconvex, convex, ulnar convex and plan. Results: VCA varied between columns of the distal radius extremity. The mean in the lateral column was 153.40 degrees and 146.06 degrees in the intermediate. Four of nine plates showed no variation in volar angulation. From 27 distal radius, 13 showed convex pattern and 12 had biconvex, whereas most plates (seven of the nine analyzed) were biconvex drawing. Conclusion: Radial bone anatomy was variable. The intermediate column was, on average, steeper than lateral column. Synthes®-2 plate presented the closest design to the anatomy of the distal end of the radial bone, followed by Newclip®-2 plate. RESUMO Objetivos: Avaliar o ângulo palmar cortical (APC) da extremidade distal do rádio, sua variação nas colunas lateral e intermédia, o formato e o padrão da watershed line (WL) e sua correlação com as placas volares bloqueadas disponíveis no mercado. Métodos: Foram analisados 27 ossos rádios do Departamento de Morfologia e nove placas de seis fabricantes diferentes. Medimos os APCs das colunas lateral e intermédia dos rádios e seus correspondentes nas placas, além de compararmos o relevo do terço distal do rádio e das placas. Quanto a WL, comparamos o formato nas peças e seu equivalente nas placas, sendo classificados em quatro tipos: biconvexo, convexo, convexo ulnar e plano. Resultados: O APC variou entre as colunas do rádio, sendo a média da coluna lateral 153,40° e, da intermédia 146,06°. Quatro das nove placas não apresentaram variação na angulação volar. Dos 27 rádios, 13 apresentaram padrão convexo e 12 biconvexo, enquanto a maioria das placas (sete das nove analisadas) tinham desenho biconvexo. Conclusão: A anatomia dos rádios foi variável entre as peças, sendo a coluna intermédia mais inclinada que a coluna lateral. A placa Synthes ® -2 foi a que apresentou relevo mais próximo dos achados anatômicos da extremidade distal do rádio, seguida pela Newclip ® -2. Nível de Evidência II, Estudo Anatômico Descritivo.

ABSTRACT

The calcifying epithelial odontogenic tumor is a rare benign neoplasm that accounts for approximately 1% of all odontogenic tumors. Most of the cases occur in the posterior mandible, and a few involve the maxilla. Despite their relatively indolent biological behavior, tumors in the maxilla tend to grow fast. We report the case of a 33-year-old female patient exhibiting swelling in the right maxilla. An isodense area associated with an impacted supernumerary tooth was found on imaging examination. The histopathologic diagnosis was a calcifying epithelial odontogenic tumor. The treatment of choice was surgical removal of the lesion and associated dental elements. The patient has been followed up for 11 months and shows no signs of recurrence. Besides describing this case, we reviewed the literature on the association of calcifying epithelial odontogenic tumors with supernumerary teeth and found two case reports addressing this subject.

Introduction: Even though syphilis is an easily detectable and treatable disease, it is still considered a major public health problem, which may lead to Congenital Syphilis (CS). Objective: To analyze the final conclusion and the situations of vulnerability of cases of CS reported in Niterói in 2018 and 2019. Methods: A review of the reported cases of CS, except for abortions, diagnosed in 2018 and 2019, was carried out through SINAN forms and research questionnaires. Results: Of the 46 cases of CS in 2018 and the 107 cases in 2019, 8 (17.4%) and 4 (4%) did not undergo prenatal care, respectively; 2 (4.4%) and 4 (4%) were not screened for CS in the prenatal period; 3 (6.5%) and 13 (12%) were screened, but not treated; 11 (23.9%) and 17 (16%) did not receive an adequate treatment; 1 0 (21.7%) and 23 (21%) were reactive in the screening process, received adequate treatment, but were reinfected; 9 (19.6%) and 32 (30%) were reactive in the screening process, received adequate treatment, but had confirmed CS due to other criteria; 3 (6.5%) and 12 (11%) were nonreactive in prenatal care, but reactive in childbirth; and 0 (0%) and 2 (2%) were reactive, considered a serological scar, untreated, but confirmed by other criteria. The "low-income family" vulnerability aspect appears 21 times in 2018 and 50 times in 2019; "alcohol user", 11 times in 2018; "frequent change of address", 18 times in 2019. Conclusion: The social context of pregnant women living in Niterói in 2018 and 2019 may have determined the outcome of congenital syphilis.


Introdução: A sífilis, embora seja um agravo de fácil detecção e tratamento, ainda é considerada um grave problema de saúde pública, podendo acarretar a sífilis congênita. Objetivo: Analisar a conclusão final e as situações de vulnerabilidade dos casos de sífilis congênita residentes em Niterói (RJ) notificados em 2018 e 2019. Métodos: Foi realizada revisão dos casos de sífilis congênita residentes notificados, exceto abortos, com diagnóstico em 2018 e 2019, por meio das fichas do Sistema de Informação de Agravos de Notificação e dos questionários de investigação. Resultados: Dos 46 casos de sífilis congênita em 2018 e 107 em 2019, temos respectivamente que 8 (17,4%) e 4 (4%) não fizeram pré-natal; 2 (4,4%) e 4 (4%) não fizeram triagem para sífilis congênita no pré-natal; 3 (6,5%) e 13 (12%) fizeram triagem, porém não trataram; outros 11 (23,9%) e 17 (16%) não fizeram o tratamento adequado; 10 (21,7%) e 23 (21%) foram reagentes na triagem, tratamento adequado, porém reinfectaram; 9 (19,6%) e 32 (30%) foram reagentes na triagem, tratamento adequado, porém confirmaram sífilis congênita por outros critérios; 3 (6,5%) e 12 (11%) foram não reagentes no pré-natal, porém reagentes no parto; 0 (0%) e 2 (2%) foram reagentes, considerados cicatriz sorológica, não tratados, porém confirmaram por outros critérios. A vulnerabilidade família de baixa renda apareceu 21 vezes em 2018 e 50 vezes em 2019, usuária de álcool (11) em 2018, mudança frequente de domicílio (18) em 2019. Conclusão: O contexto social das gestantes residentes em Niterói em 2018 e 2019 pode ter determinado o desfecho de sífilis congênita.

Abstract Objective: To investigate the association between birth weight and excess weight among students aged 6-14 years, adjusted for life course confounding factors. Methods: Cross-sectional study with 6-14-year-old schoolchildren in 2010; 795 school children from two public schools. In addition, a sub-sample was selected using a case-cohort study approach. Sociodemographic, breastfeeding, food introduction, previous weight gain, family history, current clinical and behavioral variables as well as maternal variables related to pregnancy, were collected. Multivariable weighted logistic regression was used to evaluate the association between birth weight and overweight. All prevalent cases of overweight (n = 160) were selected to compose the case group and a random sub-sample of all students participating in the study (n = 276 students, of whom 88 were cases) were the control group. Results: An unadjusted 6% increase in the excess weight prevalence ratio (p-value = 0.004) was found for each 100 g increase in birth weight. With adjustment for age, sex and behavioral variables (models 1 and 2), the association of birth weight with excess weight was positive and statistically significant, but it was no longer significant in the final model (model 3) when clinical variables were considered. Conclusions: Although some of the secondary associations were statistically significant, we could not identify a significant association between birthweight and excess weight in adolescents.

Abstract Introduction: Spondias tuberosa is a tree endemic to the semiarid region of Brazil with fruticulture potential. Objective: To estimate the diversity and genetic structure of S. tuberosa accessions from four areas of the semiarid region of Brazil, in order to facilitate conservation genetic resources studies in this species. Methods: DNA was extracted, using the CTAB 2x method, from leaf samples of 24 accessions of S. tuberosa available in the germplasm bank at Embrapa Semiárido, Brazil. Ten microsatellite loci were used in this study. Results: The UPGMA dendrogram, generated with a Jaccard coefficient similarity matrix, contains four groups at a 0.44 cutoff point. The similarity coefficient ranged from 0.30 to 0.84, indicating great divergence among the accessions. A Bayesian analysis conducted with the software Structure suggests there are two subpopulations, one formed by accessions from the Januária region and another by accessions from the Juazeiro, Uauá and Petrolina regions. The ΦST value of 0.12 for the analysis of molecular variance indicates moderate genetic differentiation among the four populations, suggesting that the genetic variability is moderately structured in function of region. Conclusions: Together, the analyses indicate that the genetic diversity of S. tuberosa is not uniformly distributed in the studied regions. Thus, germplasm from a greater number of populations should be collected to increase the germplasm bank genetic diversity of the species.


Resumen Introducción: Spondias tuberosa es un árbol endémico de la región semiárida de Brasil con potencial frutícola. Objetivo: Estimar la diversidad y caracterizar la estructura genética de accesiones de S. tuberosa en cuatro áreas del semiárido brasileño, para así facilitar estudios de conservación de recursos genéticos de esta especie. Metodología: El ADN fue extraído utilizando el método CTAB 2x a partir de muestras de hojas de 24 accesiones de S. tuberosa disponibles en el banco de germoplasma de Embrapa Semiárido, Brasil. Diez loci de microsatélites fueron usados en este estudio. Resultados: El dendrograma UPGMA generado con una matriz de similitud de coeficientes de Jaccard, formó cuatro grupos con punto de corte en 0.44. El coeficiente de similitud osciló entre 0.30 y 0.84, indicando una gran divergencia entre las accesiones. El análisis Bayesiano realizado en el software Structure sugiere la existencia de dos subpoblaciones, una formada por las accesiones de la región de Januária y otra derivada de las regiones de Juazeiro, Uauá y Petrolina. El valor de ΦST de 0.12 derivado del análisis molecular de la varianza indica moderada variación genética entre las cuatro poblaciones, sugiriendo que la variabilidad genética se estructura moderadamente en función de la región. Conclusiones: Los análisis en conjunto indican que la diversidad genética de S. tuberosa no se encuentra distribuida uniformemente en las regiones estudiadas. Por lo tanto, se debe recolectar germoplasma de un mayor número de poblaciones para aumentar la diversidad genética del banco actual de la especie.
